Cigna health plans can cover inpatient rehab for drug and alcohol addiction when the stay is medically necessary. What you pay depends on your plan type, whether the treatment center is in network, and whether Cigna approves the stay before admission.
Knowing how coverage works before admission helps you avoid surprise bills and delays in starting care.
Cigna health insurance benefits for substance abuse treatment differ from one health plan to the next, so your plan documents have the final word. To see how benefits checks work at a men’s residential program in Arizona, read our page about rehabs that accept Cigna insurance.
Does Cigna Insurance Cover Drug and Alcohol Rehab?

Cigna sells employer-sponsored health plans and individual plans, and behavioral health benefits on its plans are administered by Evernorth Behavioral Health [1]. Cigna lists inpatient and outpatient treatment services among its substance use benefits.
Those benefits apply to alcohol rehab and to drug rehab for substances such as opioids, stimulants, and sedatives. Cigna’s own consumer guide calls addiction “a chronic and progressive disease” [2].
Our page on alcohol addiction covers the signs, health risks, and treatment paths for drinking problems.
Federal Rules Behind Cigna Rehab Coverage
The Mental Health Parity and Addiction Equity Act of 2008 sets the main federal rules. For plans subject to it, addiction treatment cost sharing and numerical limits cannot be more restrictive than the predominant limits applying to substantially all medical and surgical benefits in the same category [3].
The Centers for Medicare & Medicaid Services notes that this law does not itself require a plan to offer mental health or substance use benefits. When a covered plan offers them, parity requirements apply.
The Affordable Care Act fills part of that gap. Non-grandfathered individual and small group health insurance plans must cover mental health and substance use disorder services as one of ten essential health benefit categories.
Parity also reaches care management rules such as prior authorization. A health plan cannot apply those rules more strictly to addiction treatment than to comparable medical care.
Coverage is only one part of choosing a program. Our guide on how to choose a rehab in Arizona covers licensing, levels of care, and the questions to ask an admissions team.
Why Your Health Insurance Plan Type Matters
Cigna plans come in several designs, and the design shapes your rehab coverage:
- A Cigna PPO, or preferred provider organization plan, usually pays part of the cost at out-of-network providers.
- HMO plans generally cover care only from in-network providers, outside of emergencies, and may require a referral.
- An exclusive provider organization (EPO) plan generally covers only in-network care, except in emergencies or when the plan authorizes an exception. It usually does not require referrals.
Who regulates the plan matters too. Many large employers pay claims from their own funds and hire Cigna to run the plan. These self-funded plans follow federal rules rather than Arizona insurance rules.
Your HR department or the member services number on your Cigna insurance card can tell you which kind of plan you have. Ask for the coverage details for residential treatment, which can differ from hospital benefits.
What Cigna Means by Medical Necessity
Medical necessity is the standard Cigna uses to decide whether a level of treatment for substance use fits a person’s needs. A stay is deemed medically necessary when a clinical review finds that care in a less intensive setting would not be safe or effective.
Reviewers start with how severe the substance use is. Families still sorting out whether drinking has become a problem can start with the signs of alcoholism.
How ASAM Criteria Shape Approval
Evernorth uses the ASAM Criteria, a framework from the American Society of Addiction Medicine, to guide level-of-care reviews for substance use disorders. Beginning in November 2024, it applied the 4th Edition to customers 18 and older, unless a contract or state or federal law requires other criteria [4].
For substance use disorders, reviewers consider withdrawal risk, physical and mental health, substance-related risks, the living environment, and the person’s preferences and barriers to care. ASAM guides substance use reviews; separate mental health conditions may require other clinical criteria.
Withdrawal risk carries extra weight. Heavy drinkers who stop suddenly can face alcohol withdrawal seizures and other dangerous symptoms, which is one reason a plan may approve a higher level of care.
Prior Authorization and Ongoing Reviews
Some inpatient and residential services require prior authorization. The treatment center usually submits clinical information, but you should confirm who is responsible, especially for out-of-network care. Approval does not guarantee payment; eligibility and plan terms still apply. Emergency services do not require precertification [5].
Approval often covers a set number of days at a time. The program then sends progress updates, called concurrent reviews, and the plan decides whether more days are covered. Reviewers may also ask for a treatment plan with measurable goals.
The length of a covered stay depends on ongoing clinical need, plan benefits, and review requirements. Approval of an initial stay does not guarantee every later day, and a reviewer may recommend a different level of care.
Treatment Options Cigna Covers, From Detox to Outpatient
Cigna’s consumer guide describes several levels of care for substance use disorders. Each level has its own approval rules and costs.
Medical Detox
Medical detox manages withdrawal with clinical monitoring and medication when needed. Its length and setting depend on the substance, symptoms, and health risks. Withdrawal management may occur in an outpatient, residential, or hospital setting and can overlap with ongoing addiction treatment.
Detox focuses on withdrawal stabilization, while ongoing addiction treatment addresses substance use, mental health, and recovery skills. Our post on what the first few weeks after quitting alcohol feel like explains how symptoms can change after the first days.
Residential Treatment
Residential treatment means living at a treatment facility while receiving counseling and other care. It differs from hospital inpatient treatment, which provides more intensive medical services. Cigna’s consumer guide says residential treatment may last 28 days or more [2], but that is not a guaranteed covered stay.
Residential treatment can suit people who need a structured living environment and ongoing addiction care. The program must be equipped to meet their medical and psychiatric needs. Into Action’s inpatient rehab in Phoenix is a residential program; confirm how your plan classifies and covers it.
Partial Hospitalization and Intensive Outpatient Programs
Partial hospitalization programs meet five to seven days a week for six to eight hours a day, according to Cigna’s guide. Intensive outpatient programs usually meet three to five days a week for about three to four hours a day. Standard outpatient services, such as weekly counseling, are the least intensive level.
Outpatient treatment lets a person live outside the facility, although intensive schedules can affect work and family commitments. Whether Cigna approves inpatient or outpatient treatment depends on the clinical review and plan benefits, and people may move between levels as their needs change.
Co-Occurring Disorder Treatment and Medication
Cigna plans may cover co-occurring disorder treatment, which addresses addiction alongside conditions such as depression, anxiety, or post-traumatic stress disorder. Covered services and approval requirements vary by plan. Our guide to co-occurring disorders explains how these conditions can interact in men.
Medication-assisted treatment pairs FDA-approved medications with counseling. Depending on the plan, the medication may fall under the medical benefit or the prescription drugs benefit.
What You May Pay for Inpatient Rehab With Cigna
Your share of treatment costs usually includes a deductible, then coinsurance or a copay until you reach your out-of-pocket maximum. Plans that follow the Affordable Care Act cap that yearly amount for in-network covered care.
On individual plans, bronze tiers usually have lower premiums and higher deductibles than gold or platinum tiers. A person on a bronze plan may pay more of a residential stay before coinsurance begins.
In-Network vs. Out-of-Network Benefits
Using in-network providers usually costs far less, because Cigna has agreed rates with those health care providers. Out-of-network services may cost more, need pre-certification, or have no coverage at all on HMO and EPO plans.
On PPO plans, out-of-network benefits often carry a separate deductible. Those expenses generally do not count toward the in-network maximum. For planned out-of-network rehab, the facility may also bill you above the plan’s allowed amount; confirm any applicable billing protections or network exception.
Some men still go out of network for a specific approach, such as gender-specific recovery for men. In that case, ask the program for a written cost estimate before admission.
Single Case Agreements
A single case agreement sets payment terms for a specific person’s care with an out-of-network provider. A plan may approve an exception when its network cannot meet the person’s clinical needs, but in-network cost sharing is not automatic.
Ask the insurer and treatment center to confirm any agreement in writing before admission, including the services, dates, payment rate, your share of costs, and whether the provider can bill you above that amount.
How to Check Your Cigna Insurance Benefits
Checking coverage before admission can help prevent surprise bills. You can call your insurance provider at the number on your card, or treatment centers that accept Cigna insurance can run the check with your information. Ask these questions:
- Is residential addiction treatment covered, and does it need prior authorization?
- Is the treatment center in network, and if not, what are my out-of-network benefits?
- What are my deductible, coinsurance, and out-of-pocket maximum, and how much have I met this year?
- Are partial hospitalization, intensive outpatient programs, and detox covered as separate levels of care?
Write down the date, the representative’s name, and a reference number for each call. Admissions teams also ask about substance use history, and our overview of the stages of addiction can help you describe how use has changed over time.
What to Do if Cigna Denies Inpatient Rehab

A denial can be appealed. Some denials turn on missing paperwork or on how the clinical picture was described.
Internal Appeals
A denial letter should state the reason, such as medical necessity or a missing authorization. Ask the treatment center and your doctor to help gather records that answer that specific reason. Keep copies of every letter, record, and call note.
For urgent situations, ask for an expedited appeal. Plans must decide urgent appeals faster than standard ones, and the letter explains the deadlines for your plan.
External Independent Review in Arizona
For Arizona-regulated plans, external review is generally available after required internal appeals are completed or deemed exhausted. Submit the request to the insurer within four months of receiving the applicable written denial [6]. For qualifying expedited medical reviews, the independent reviewer has 72 hours after receiving the case from DIFI to decide; referral and notification take additional time.
Self-funded employer plans are not eligible for the state process and follow their own federal appeal steps. Your denial letter explains which appeal path applies to your plan.
Does Cigna Cover Inpatient Rehab? Frequently Asked Questions
What if I Am Calling About Cigna Coverage for a Family Member?
Family members can ask general questions, but access to an adult member’s private information generally requires that member’s permission or legal authority. Ask Cigna about its authorization process. We also explain how to support someone without enabling him in our post on helping vs. enabling here.
How Long Does Insurance Pay for Inpatient Rehab?
There is no single covered length of stay across Cigna plans. Approval may cover an initial period, followed by reviews of ongoing need. Check both your benefit terms and the authorization, and ask how extensions, denials, or transitions to outpatient care will be handled.
Does Cigna Cover Outpatient Rehab?
Cigna lists outpatient substance use treatment among its possible benefits, including care described in its guide such as partial hospitalization and intensive outpatient programs. Your own coverage, costs, network requirements, and any prior approval depend on the plan. Confirm those details before scheduling treatment.
Does Cigna Cover Medical Detox?
Cigna plans may cover medically necessary withdrawal management, subject to plan benefits and authorization rules. The appropriate setting and length depend on clinical risk. Into Action Recovery does not operate a detox center; if you need withdrawal care, confirm an appropriate provider and its coverage before admission.
Can I Use Cigna at an Out-of-State Rehab?
It depends on your plan. A Cigna PPO may pay part of the cost at an out-of-state treatment center, either in-network or out-of-network. HMO and EPO plans generally limit coverage to the Cigna network, so confirm network status before you travel for treatment.
Check Your Cigna Benefits
A benefits check can clarify your plan’s coverage and estimated costs before admission. You can verify your insurance benefits for free, and an admissions coordinator can help explain the results. Verification is an estimate, not a guarantee of approval or payment.
