What Cigna Rehab Coverage Includes

Most Cigna plans provide insurance coverage across the full continuum of addiction treatment. Cigna recognizes several distinct levels of care:

Medical detox

Supervised withdrawal management with 24-hour monitoring. Outpatient detox is covered under some plans when clinical risk is lower.

Inpatient rehab

Alcohol Rehab and Cigna Insurance

Alcohol rehab is the most common reason men call us, and Cigna coverage for alcohol rehab follows the same substance use disorder benefit that covers every other drug.
Alcohol rehab almost always begins with medical detox, because alcohol withdrawal carries seizure risk that most substances do not. From there, alcohol rehab moves into residential treatment, where the clinical work actually happens. Cigna covers both stages when medical necessity supports them, and alcohol rehab authorizations tend to be straightforward because the withdrawal risk is well documented.

Cigna Plans: PPO, HMO, and EPO

Your plan type shapes which treatment centers you can choose, and most men have no idea which they carry until they check the card.
Cigna PPO plans. A preferred provider organization includes out of network coverage. Cigna PPO plans allow out of network treatment options, meaning you can receive care from an out of network provider and your plan still pays a real percentage. This is why many men choose a program on clinical fit rather than a directory listing, and most men who reach Into Action arrive holding a PPO.
HMO plans. A health maintenance organization generally requires in network providers and a referral from your primary care physician. Out of network care is usually limited to emergencies.
EPO plans. An exclusive provider organization sits between the two. Cigna network required, referrals usually not.
Cigna offers employer sponsored health plans, individual marketplace coverage, and Medicare plans. Cigna Medicare Advantage plans carry additional benefits beyond Original Medicare, though authorization rules and networks differ from commercial coverage. We verify all of them the same way.

How Out of Network Benefits Work in Your Favor

This is the section that saves men the most money, and almost nobody explains it.
Out of network services are not uncovered services. Men rule out treatment centers they could have afforded because they assume those two phrases mean the same thing. They do not.
Under a Cigna PPO, out of network coverage means a separate deductible and a different coinsurance split, but Cigna still pays toward your treatment. A man with strong out of network benefits frequently pays less at an out of network facility that fits him than at an in network facility that does not. Out of network treatment can increase your out-of-pocket costs compared to in network care. It does not remove your Cigna coverage.
Three things determine what out of network care actually costs you:
Your out of network deductible. Separate from, and usually higher than, your in network deductible.
Your coinsurance split. In network care might run 80/20. Out of network services might run 60/40.
Whether your plan carries out of network benefits at all. PPO plans do. HMO plans frequently do not. This is the first thing we check.
One detail men miss: Cigna may require pre-certification for out of network services specifically, separate from standard prior authorization. Skipping it is a common and expensive mistake. We handle that step for you.

There is a fourth path most men never hear about.
When no in network facility meets a documented clinical need, Cigna may approve an out of network provider at in network rates. That arrangement is called a single case agreement, and it is decided case by case on clinical justification.
A men’s-only residential program is a legitimate basis for that argument. So is specialized trauma care, or a documented history of failed treatment in mixed-gender settings. It is not automatic, but it is real, and a treatment facility that never raises it with Cigna on your behalf is leaving your money on the table.

Medical Necessity and Prior Authorization: We Handle It

Cigna authorizes inpatient or outpatient treatment based on medical necessity. A clinician must document that a lower level of care would not be safe or effective for you. Cigna insurance coverage varies based on medical necessity and the specific treatment type requested.
The review weighs withdrawal risk, medical and psychiatric stability, treatment history, relapse potential, and whether your home environment supports recovery. This is why two men with identical Cigna health insurance receive different authorizations.
Prior authorization is required for certain levels of care under most Cigna plans, residential treatment among them. You do not have to manage any of it.
