Content on the Site is not medical advice and does not create a provider-patient relationship. Submitting a form or verifying benefits does not constitute admission to treatment. Eligibility
You must be at least 18 years of age to submit a form on the Site. By submitting a form, you represent that you are 18 or older and that the information you provide is accurate.
Phone & SMS/Text Message Communications
By checking the phone consent box on any form on the Site, you agree that Into Action Recovery may call you at the phone number you provided to respond to your inquiry and discuss our programs and services. Phone consent is required to submit a form.
By checking the optional SMS consent box, you agree to receive recurring text messages from Into Action Recovery, including appointment reminders, updates, and service-related messages, at a frequency of approximately 2 to 4 messages per month. Consent to receive text messages is not a condition of receiving treatment or any goods or services. Message and data rates may apply.
You may cancel text messages at any time by replying STOP to any message. You will receive one confirmation message and no further texts unless you re-enroll. Reply HELP for assistance, or contact us using the information on this website. Text messages are supported by major U.S. carriers. Carriers are not liable for delayed or undelivered messages.
You represent that you are the owner or authorized user of the phone number you provide. If you change or deactivate that number, you agree to notify us so we can update our records. We do not share your mobile number or consent with third parties or affiliates for marketing purposes. Our handling of your information is described in our Privacy Policy.
Insurance Verification
Verifying your benefits through the Site is free and carries no obligation. Verification results are estimates based on information provided by you and your insurer and do not guarantee coverage, admission, or final cost.
