Aetna insurance plans sold on the individual market, and most employer-sponsored health insurance plans, must include coverage for addiction treatment. How Your Aetna Plan Type Affects What You Pay
Aetna insurance plans come in several structures, and the structure shapes which treatment centers you can choose from.
PPO plans carry out of network benefits, which means you can attend treatment at rehab centers outside the network and Aetna still pays a substantial portion. Most of the men who come to Into Action arrive on a PPO, and many pay less than they expected.
HMO plans generally require in network providers and may route you through a primary care physician.
EPO plans sit between the two: network required, referrals usually not.
Into Action works with Aetna members through out of network benefits and, where the clinical picture supports it, single case agreements. A single case agreement is an arrangement where the insurance provider authorizes an out of network rehab facility at in-network rates because no in-network option meets the documented need. A men’s-only residential program is a legitimate basis for that argument, and we make it on your behalf when it applies.

Aetna Medicare Plans and Inpatient Treatment
If you carry an Aetna Medicare Advantage plan, the rules differ. Medicare Part A covers medically necessary inpatient rehabilitation, and Aetna administers those benefits under its Medicare products. Aetna provides coverage for rehabilitation services under most commercial and Medicare plans, though authorization requirements and networks are not identical.

Drug Rehab Coverage and Alcohol Rehab Under an Aetna Plan
Does Aetna cover drug rehab and alcohol rehab? Yes, and Aetna covers rehab for both under one category: substance use disorder benefits. There is no separate benefit for cocaine versus alcohol versus opioids. What changes is the clinical picture and the level of care your assessment supports.
Alcohol rehab often begins with medically supervised medical detox because alcohol withdrawal carries seizure risk. Opioid treatment frequently involves medication assisted treatment alongside therapy. Stimulant use disorder usually moves straight into residential treatment. Aetna drug rehab coverage follows the clinical need.

Behavioral Health Services, Mental Health, and Co-Occurring Disorders

Mental health services and substance use treatment belong in the same building, on the same schedule.

Medical Necessity and Prior Authorization: We Handle It
Aetna authorizes inpatient treatment when a clinician documents that a lower level of care would not be safe or effective. That documentation is called medical necessity, and it determines whether inpatient rehab programs get approved and for how long.
Aetna requires prior authorization for inpatient and residential treatment under most plans. The treatment facility submits the paperwork, Aetna evaluates medical necessity during that review, and denials commonly happen when prior authorization was never obtained or the clinical case was documented poorly.

Rehab Cost: What You Pay Out of Pocket
Four numbers on your Aetna plan determine your out of pocket costs: the deductible, the coinsurance percentage, the out-of-pocket maximum, and network status.
